4. Print the form, fill in the details as mentioned in the form.

5. Alongwith the form kindly send Demand Draft of Rs. 1,250/- in favour of MFPA Artists Pvt. Ltd.
and mail / courier it to MFPA Artists Pvt. Ltd., Maker Bhavan No.1, 2nd Floor,1 New Marine Lines,Mumbai - 400020 (INDIA)

6. Once your payment has realised in our bank, you will receive a Kit alongwith your form by courier.
